Using Double Wires as a Stability Solution

At times, when the endovascular delivery or catheter system is unstable, two wires can be used, forming a double wire system.

Placing a second 0.018 Fr wire, instead of changing to a single 0.035 Fr wire for stability, can help when faced with extreme angulations. This technique is displayed below (Figure 1) where stent graft alignment was needed after graft separation in a complex aortic case. This is a simple method, and the limitation is usually the catheter size and the types of wires and catheters that are available. Double wires can be used in both acute and elective cases, and the use of steerable sheaths is very helpful in these situations.
